- Legal status: Tramadol is not a scheduled substance under the US Controlled Substances Act, it is available with a normal prescription.
Analgesic effects of Tramadol hcl involve a complementary dual mechanism of action: mild µ-opioid receptor binding (like opioid pain killers) and inhibition of norepinephrine and serotonin reuptake (like antidepressants).
Tramadol hcl is used to treat a wide range of painful conditions:
- acute musculosketetal pain
- dental pain
- osteoarthritis
- neuropathic pain
- posttraumatic pain
- postoperative pain
- labour pain, obstetrical analgesia
- renal colic
- diabetic neuropathy pain
- postherpetic neuralgia

Tramadol may be taken without regard to meals. Ultram ER extended release tablets should be taken consistently: always with food or always on an empty stomach.
The usual dosage for pain management is 50-100 mg every 4-6 hours. You should not exceed 400 mg per day.
Do not increase dose or frequency of tramadol without doctor's approval.
Extended use of Tramadol hcl (Ultram) may cause physical and psychological dependence. Avoid abrupt discontinuation and taper dose when discontinuing to avoid withdrawal symptoms.
When prescription for tramadol (Ultram) is inappropriate:
- Hypersensitivity to tramadol or other opioids
- History of substance abuse
- Intoxication with alcohol, hypnotics, opioids, or psychotropic drugs
- During or within 14 days following MAO inhibitor therapy
